The Acknowledgment of Paternity Affidavit is for women who are not married at the time of the child's birth, or not married within 300 days from the date of conception. This document is generated from either the hospital at the time of the child’s birth, or after the birth at a local health department or a Child Support Enforcement Agency (CSEA). If the mother is married and the husband is not the father of the child, the mother and the biological father can contact the CSEA for genetic testing. The CSEA has the authority to order Vital Statistics to remove the husband's information based on their findings, and put the biological father's information on the birth certificate.
